A type 1 diabetic, Father and grandfather is diagnosed with Parkinson’s and dementia. After having a heart attack his family decides to have full-time care put in place. His family is scattered around the state, but all want to be involved. He has lived between NY and Florida for years and has multiple doctors in each place- each with their own medical records system. All his services need to be addressed in his new home.
